Healthcare Advocacy
During or after a hospital admission.
We assist you with ensuring your post-acute care needs are arranged and ready to go once you discharge from the hospital. We will continue to check in with you to ensure all services meet your needs in the home setting. We can also develop a new plan for services when you require a higher level of care.
Senior Placement
In a community to meet and fit your individual needs. This is no cost to you!
We complete a needs assessment and determine what care home or assisted living facilities best fit your needs and budget. We take this work and research off your plate and provide you with local options to choose from. This service is 100% free!
Linkage to Community Resources
to ensure safety such as arranging caregiving services and/or applying for healthcare waiver programs when needed.
Comprehensive Safety Assessments
Conducted via client's home or through virtual meeting and recommendations for services. Assessments can be offered to established healthcare agencies or individuals.
Facility and Check-ins Advocacy
(via telephone, virtual or in-person)
We can do regular or quarterly visits with you at your long-term care placement. We ensure that the facility is meeting your needs and will continuously serve as your advocate no matter where you are located.
